Bridgwater Health and Wellbeing Coach (families)

we are looking for a full time or two part time enthusiastic Health and Wellbeing Coaches who share our passion for promoting active, happy, healthy and resilient families. Funded through the Bridgwater Bay Primary Care Network, you will work closely with targeted families and the wider primary and secondary care services as well as the wider SASP team to establish positive changes with the families, to improve their health and wellbeing.

About Bridgwater Health and Wellbeing Coach (families)

The NHS Long Term Plan established clear objectives for health systems to deliver improvements in health inequalities that have been a persistent feature of the social landscape of England. Working with primary care to identify health inequality challenges faced by families with younger parents in Bridgwater you will work collaboratively to support and empower the lives of families through sport and physical activity as a means of securing better physical and mental health. We wish to create local conditions to get more young families active and use this to improve their health and wellbeing and social connectedness.

To support this work we are looking for a full time or two part time enthusiastic Health and Wellbeing Coaches who share our passion for promoting active, happy, healthy and resilient families. Funded through the Bridgwater Bay Primary Care Network, you will work closely with targeted families and the wider primary and secondary care services as well as the wider SASP team to establish positive changes with the families, to improve their health and wellbeing. You will work with both individuals, groups and partner organisations to support happier and healthier environments for those you work with.

This project is one of the NHS 20 Complex Community Care Demonstration Projects and will have national interest in its delivery processes and results.
